Make PBMs Irrelevant...
 
If you were making 50% of your revenue from non-PBM sources, how do you think it would change your PBM stress levels? 
 
I bet it would it would make them all but disappear. 
 
When you have 90++% of your revenue that goes through a PBM, it makes for a tough life as a pharmacy owner. 
  • Profit is low
  • Stress is high
  • Poor cash flow
  • Joy of ownership is sucked dry

🎁 My Top 5 Favorite Non-PBM Revenue Strategies
 
I could write a book solely on non-PBM revenue generation (hmmm, maybe an idea!). I think half of our PBU membership content is directed at this. Instead of blasting you with a ton, here are my top picks because 1. they are easy 2. they work. 
 
  1. Direct bill worker's compensation prescriptions through StreamCare. Bypass the PBMs, work comp insurance pays the same, you make a boatload more money (because there's no PBM to steal it all). Available in most states, but not all.
  2. Animal medications are a booming cash industry and you should get a piece of that pie. Animal Med Express is a fantastic resource to source medications and OTCs for all your patients furry or feathered friends. 
  3. Allergy testing and sublingual treatment from Wyndly is a great service they are unlikely to find elsewhere. Getting int an ENT can take months in some areas and injection allergy treatment is expensive, painful, and takes 5 years! Offer your patients a better way and earn some easy profit too. (bundle with pet medications as most owners have a pet allergy!)
  4. Anything weight loss related. From GLP-1s to supplements, weight loss is a sure winner. Offer your patients short-supply Semaglutide and Tirzepatide. BPI Labs will get you the lowest per milligram cost. Contact John Georges at 310-779-4996 or [email protected].
  5. RPM & CCM services. While these don't fall in the easy category to get started, they are so worth the effort. ValueCare Suite will make it as easy as possible, they do all the heavy lifting and you get to provide the care while your patients benefit.
 
Make a 2025 goal for non-PBM revenue. Maybe you want to get to 10% or even try for 25%. No matter where you are starting, you will benefit greatly by increasing this metric.

Happy Stories From 2024
 
While 2024 was the toughest year for independent pharmacy owners in the past 20 years IMO, there were some still great wins to have. 
 
I am sharing with you some wining stories from fellow pharmacy owners and how you can replicate them in your own pharmacy. 
 
🎁 For Our 6th Day of Christmas, You Get What's Working For Other Owners...
 
#1 - I have added thousands a month in profit by doing the supplies you suggested. Branded GLP-1s and other injectables were generally big losers in 2024. A great way to combat the loss is to add higher margin supplies (pen needles, alcohol swabs, & others) to these prescriptions. Your patients get all the tools they need and you get to not lose money! I recommend Astor Drugs for a wide variety of supplies for injectable, inhalation, and other products. 
 
#2 - I just made over $6,000 on that 1 Rx... I might end the year up! Yes, there are still some high margin prescriptions out there. Sometimes just doing 1, is all your pharmacy needs like this owner in Nebraska. Many high margin products are not available from the big 3. Instead, they are available from the secondary wholesaler market such as Axia, Wellgistics, Keysource, GRx, Real Value Rx, Prodigy Health, and others. My favorites include Oxaprozin 300mg, Nitrivia, Ziphex, and Xybiotic. 
 
#3 - I had no idea how much money I was giving up by billing regularly. This feedback was from another PBU member who didn't realize that by direct billing worker's compensation scripts, they could be making a lot more money. This pharmacy quickly switched their billing and profited an extra $10,000 a month for just 12-15 work comp patients. If you aren't direct billing then you are giving the PBMs thousands of dollars of YOUR hard-earned money. My favorite for making direct billing easy is StreamCare. Don't let thousands slip through your fingers. 
 
#4 - I just realized my cash prices have been set too low. Lots of pharmacy owners are focusing on building up cash prescription revenue. This tactic even has its own support call inside the membership. Setting the right cash prices can feel overwhelming and can lead to lost profit if not set correctly. Once this member signed up with AxeRx, he was able to increase the profitability while remaining competitive. If you want a no brainer for cash prices, I recommend AxeRx cash discount card. Unlike the BadRx card, they won't sell your patients data and you won't lose money.
